All claims to Icon should be submitted on this form. Please read the Guidance tab (second page in the Excel file) before completing the form. Please fill in the form on your computer (not by hand) and submit as .xlsx format which our system reads best.

Please note that attachements (receipts or othere evidence of payment) must be in PDF format not jpegs as the iCompleat software cannot read jpegs.

• Icon Group Treasurer for Group activities
• Meeting organiser for the Board of Trustees, all Board Committees and Task and Finish Groups
